Responsibilities
  • Evaluate and plan physical therapy services for patients in their homes
  • Implement physical therapy plans and work with patients to achieve their goals
  • Collaborate with other healthcare professionals as part of an interdisciplinary rehabilitation team
Requirements
  • Graduate of a state-approved school of physical therapy accredited by the American Physical Therapy Association
  • Minimum of 6 months acute care or rehabilitation center experience
  • Licensed as a Physical Therapist in the Commonwealth of Pennsylvania
  • Reliable, independent means of transportation to make home visits
