Job Title: Licensed Practical Nurse

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Mineral Point Health Services. As a LPN, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare team.

Job Summary:

Under the supervision of the RN Unit Manager or DON, the LPN will be responsible for providing nursing care to assigned residents/patients, implementing and evaluating their plan of care, and supervising the performance of nursing assistants. The ideal candidate will possess excellent communication and interpersonal skills, be able to work effectively in a fast-paced environment, and demonstrate a commitment to delivering exceptional patient care.

Responsibilities:
  • Provide direct patient care, including administering medications, performing treatments, and providing emotional support to residents/patients and their families.
  • Implement and evaluate individualized plans of care, ensuring that patient needs are met and goals are achieved.
  • Supervise and direct the activities of nursing assistants, ensuring that they are providing high-quality care and adhering to center policies and procedures.
  • Collaborate with the interdisciplinary team to develop and implement patient care plans, ensuring that all aspects of care are addressed.
  • Participate in quality improvement initiatives, identifying areas for improvement and contributing to the development of solutions.
Requirements:
  • Current licensure as an LPN in the state of employment.
  • Current CPR certification.
  • Ability to use necessary supplies and equipment, including feeding pumps, oxygen, treatment trays, and medication carts.
  • Ability to calculate drug dosages and administer medications safely and accurately.
  • Ability to read, write, speak, and understand English, as well as demonstrate basic computer knowledge and ability.
