Job Summary

We are seeking a highly skilled and compassionate Family Nurse Practitioner to join our team at Elevance Health. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and contributing to the delivery of high-quality healthcare services.

Key Responsibilities
  • Administer ordered therapies according to prescriber order, facility protocols, and company policies and procedures.
  • Complete admission processes as directed by the Center Director.
  • Perform physical assessments on patients as warranted and for new consults as warranted by the patient's diagnosis.
  • Document all pertinent data in the patient's medical record.
  • Report significant changes or observations to the Center Director and follow up on any problems identified.
  • Communicate effectively with other members of the IV team on patient status and observations.
  • Maintain patient confidentiality at all times by adhering to HIPAA laws and regulations.
  • Evaluate patient responses to therapy and document findings in the medical record.
  • Educate patients and/or family members regarding therapy plans.
  • Participate in the Quality Improvement program.
  • Ensure that all medication orders are complete, accurate, and up-to-date prior to treatment.
  • Participate in the On-Call program, which may include after-hours, weekend, and holiday calls and visitations.
  • Provide clinical coverage for nurse practitioners on PTO/LOA and support new market launches.
Requirements
  • Requires an MS in Nursing and a minimum of 2 years of related experience with clinical emphasis for nurse practitioner; or any combination of education and experience that would provide an equivalent background.
  • Experience with IVs is required.
  • Current, active, and valid unrestricted NP license to practice as a healthcare professional with scope of license in applicable state is required.
  • Requires active, current, and valid Family Nurse Practitioner Certification.
  • Multi-state licensure is required if this individual is providing services in multiple states.
  • Satisfactory completion of a Tuberculosis test is a requirement for this position and Hepatitis B vaccine or signed waiver.
Preferred Skills and Experiences
  • Healthcare experience with IVs is strongly preferred.
  • 2+ years of experience with IVs is preferred.
  • Port, PICC, and Peripheral Line experience is preferred.
  • Medication Mixing experience is preferred.
  • Titration experience is preferred.
