About the Position

As a Nurse Practitioner or Physician Assistant, you will conduct adult/geriatric assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.

You will work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.

Responsibilities include:

  • Conducting assessments and providing care to patients in their homes, skilled facilities, and through telehealth visits
  • Collaborating with primary care physicians on patient education and follow-up
  • Providing services in a variety of venues, including home visits, skilled facility visits, and telehealth visits
Requirements

To be successful in this role, you must have:

  • A Master's degree in Nursing or a related field, or commensurate experience and satisfactory completion of NP licensure
  • Current RN and NP or PA licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority
  • Board certification by the NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialty
  • A minimum of 1 year of experience as a Nurse Practitioner or Physician Assistant
  • Strong computer skills and familiarity with EMR software
  • Excellent verbal and written communication skills
Travel Requirements

You will be required to travel to meet patients in their homes, skilled facilities, and other community environments. A valid state driver's license and adequate automobile insurance coverage are required.
