Responsibilities- Conduct thorough assessments of patients' nutritional needs and develop personalized care plans.
- Collaborate with interdisciplinary teams to ensure seamless patient care and coordinate with diet clerks to ensure accurate diet orders.
- Provide education and nutrition counseling to patients, families, and caregivers on diet modifications and food-drug interactions.
- Accurately assess patients' nutritional status and determine appropriate nutritional diagnoses.
- Establish measurable nutrition intervention plans and provide follow-up reassessments to evaluate their effectiveness.
- Document all interventions, assessments, education, and care plans in the medical record according to approved content and format.
- Address discrepancies in diet orders and contact physicians with questions or recommendations regarding nutritional care.
- Assist in the development of monitoring clinical performance indicators for the department.
- Evaluate patient tolerance to therapeutic diets and maintain pertinent patient data to recommend or modify diets as needed.
- Approve supplements and develop a hospital formulary appropriate to the patient population.
- Maintain regular attendance and perform other duties as assigned or required.
- Registered Dietitian required (CMS requirement).
- Current Texas licensure required.
- One year healthcare experience desirable.
