Job Responsibilities:

  • Manage a caseload of approximately 10 patients per day
  • Handle patient populations within Oncology in an outpatient setting
  • Assist with DNR forms and advance directives
  • Serve as a point of contact, organizing transportation to and from the clinic
  • Complete FMLA and work disability forms
  • Facilitate dental clearances and DME requests
  • Coordinate nutrition services
  • Arrange home health and hospice referrals
  • Report directly to the social work manager/supervisor onsite
  • Business casual dress code is required
  • Orientation period depends on start time, typically lasting a couple of weeks
  • No weekend or holiday work requirements
