As a key member of our healthcare team, you will provide nutrition assessment, intervention, education, and counseling to patients of all ages to ensure their diets align with their health status and support optimal physical health and well-being.
Job SummaryUnder the direction of the Medical Director, you will conduct comprehensive assessments of patients' nutritional status and diet history, screen for potential risks, and develop counseling interventions to address specific health risks or conditions. You will also provide culturally competent education to patients and their families on nutritional needs and diets, collaborate with multidisciplinary care teams, and monitor tolerance and effectiveness of nutritional regimens.
Responsibilities- Conduct comprehensive assessments of patients' nutritional status and diet history
- Screen for potential risks and develop counseling interventions to address specific health risks or conditions
- Provide culturally competent education to patients and their families on nutritional needs and diets
- Collaborate with multidisciplinary care teams and monitor tolerance and effectiveness of nutritional regimens
- Teach nutrition classes in the center's kitchen to enable patients to make healthy food choices and manage symptoms of disease or chronic conditions
- Implement specialized evidence-based curriculums on nutrition for toddlers, diabetes, and other health conditions
- Prepare customized programs for patients to use at home and monitor progress with patient and care team
- Screen for food insecurity and link families with food and nutritional resources in the community
- Coordinate special projects to address food insecurity within the population served
- Provide occasional community presentations on nutrition and diet as it relates to specific health conditions
- Bachelor's degree and coursework accredited or approved by the Accreditation Council for Education in Nutrition and Dietetics
- Diabetes or pediatric certification preferred but not required
- Four years of related work experience preferred
- Bilingual Spanish speaking preferred
- Excellent written and oral communication skills, as well as interpersonal and team building skills
- Competency in working with diverse cultural, racial, and ethnic groups required
- Self-motivated and directed
