Job Summary:
We are seeking a Registered Nurse to provide clinical support within senior living facilities in Central Washington. This hybrid role combines on-site assessments with remote administrative tasks, requiring flexibility and adaptability.
Key Responsibilities:
- Conduct comprehensive resident assessments, identifying care needs and gaps.
- Develop and implement chronic condition care plans in collaboration with the clinical team.
- Coordinate care across multiple stakeholders, including facility teams and medical groups.
- Maintain accurate documentation in both electronic health records (EHR) and electronic medical records (EMR).
- Provide clinical expertise and support on-site staff for high-quality resident care.
Qualifications:
- Active Registered Nurse (RN) license in Washington.
- Valid driver's license.
- Strong background in geriatric care, focusing on care assessments and coordination.
- Experience in care planning for chronic conditions, including interdisciplinary team collaboration.
