Job Summary


Theoria Medical is seeking a highly skilled and compassionate Physician to join our team of healthcare professionals. As a Physician at Theoria Medical, you will be responsible for providing high-quality, patient-centered care to our residents in skilled nursing facilities. You will work closely with our Nurse Practitioners and Physician Assistants to manage day-to-day resident care, collaborate on patient diagnoses, and develop comprehensive treatment plans.


Responsibilities



  • Provide primary care services to residents in skilled nursing facilities, including diagnosing and treating acute and chronic illnesses.
  • Collaborate with Nurse Practitioners and Physician Assistants to manage resident care, including developing treatment plans and providing ongoing care.
  • Participate in Theoria Medical's Chronic Care Management, Transitional Care, RPM, and other medical quality initiatives and programs.
  • Supervise and support our Nurse Practitioners and Physician Assistants in their roles, providing guidance and feedback as needed.
  • Utilize electronic and online medical tools, including EHR systems and state/county medical systems, to document patient information and complete administrative tasks.

Requirements



  • MD or DO degree from an accredited medical school.
  • Current state licensure and board certification in Family Medicine or Internal Medicine.
  • 2+ years of experience in primary care, preferably in a skilled nursing facility setting.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a strong focus on delivering high-quality patient care.
