Job Description:

We are seeking a highly skilled Registered Nurse Case Manager to join our team at Interim HealthCare of New Hampshire. As a Registered Nurse Case Manager, you will be responsible for managing a defined caseload of patients, providing direct patient care, and comprehensive nursing services to individuals in the community.

Responsibilities:

  • Manage a caseload of patients, providing direct care and comprehensive nursing services
  • Assess patient needs and develop individualized care plans
  • Collaborate with healthcare teams to ensure seamless care coordination
  • Provide education and support to patients and their families
  • Document patient progress and outcomes

Requirements:

  • Valid nursing license in the state of NH
  • 2+ years of experience in skilled care preferred
  • 1+ year of experience in nursing supervision preferred
  • Recent wound care experience
  • Ability to lift 50 LBS with no restrictions
  • Current CPR certification
  • Reliable transportation
