Hospice Registered Nurse

We are seeking a skilled Registered Nurse to join our Home Health team at Carle Foundation Hospital. As a Hospice Registered Nurse, you will provide professional nursing care to assigned clients in support of their physician and pursuant to the objectives and policies of the Home Health Services Department.
Key Responsibilities- Provide high-quality patient care to clients of all ages and groups
- Conduct client assessments and complete visit documentation according to MMCI HHSD policy/procedure and regulatory bodies' requirements
- Develop, implement, evaluate, and amend client Plans of Care (POC) in cooperation with the physician, client/caregiver, and other healthcare members involved in the care
- Implement nursing interventions to meet client needs and changing conditions in accordance with HHSD policy/procedures
- Communicate with physicians, clients/caregivers, and other healthcare professionals involved in the care of clients per HHSD policy/procedure
- Registered Professional Nurse (RN) License upon hire
- Basic Life Support (BLS) within 30 days
- 1-3 years of experience in an acute care setting
