RN Case Manager IV

We are seeking a highly skilled RN Case Manager IV to join our team at Presbyterian Healthcare Services. As a key member of our care coordination team, you will be responsible for providing clinically-based case management to support the delivery of effective and efficient patient care.
Key Responsibilities- Integrate utilization management, care coordination, and transition planning functions to ensure seamless patient care.
- Develop and implement effective care plans to meet patients' needs, manage length of stay, and promote efficient resource utilization.
- Collaborate with the interdisciplinary team to support physicians in facilitating patient care, enhancing quality of patient outcomes, and promoting patient satisfaction.
- Provide timely and accurate information to payers to ensure optimal reimbursement.
- Registered Nurse with a Master's degree in nursing (MSN) and five years of utilization review or case management experience, or a Registered Nurse with a Bachelor's degree in nursing (BSN) and ten years of utilization review or case management experience.
- Current State of New Mexico Registered Nurse licensure required.
- Two years of clinical nursing experience in a relevant clinical practice area.
- National Case Management certification preferred.
