Job Title: Case Manager

CareStar is seeking a skilled Case Manager to provide case management services to individuals in Johnson and surrounding counties. As a Case Manager, you will be responsible for assessing the health and long-term care needs of each participant, developing and implementing individualized care plans, and authorizing services to meet their needs.

Key Responsibilities:
  • Conduct comprehensive assessments of participants' health and long-term care needs
  • Develop and implement individualized care plans that meet participants' needs and goals
  • Authorize services, including home visits and monitoring, to ensure participants receive necessary care
  • Collaborate with healthcare providers, family members, and other stakeholders to ensure comprehensive care
  • Stay current with continuing education and utilize knowledge base of Medicare, Medicaid, insurance, and state-administered waivers
Requirements:
  • Bachelor's degree in Social Work, Psychology, Sociology, Counseling, Gerontology, or Nursing
  • One year of direct service experience with the elderly or persons with disabilities
  • Effective organizational and communication skills
  • Ability to work flexible schedule, including reliable transportation and valid driver's license
