Job OverviewRehabilitation Associates, a leading multispecialty group in New England, is seeking a BC/BE Internal Medicine, Family Medicine, or Geriatrics Fellowship trained physician to join our dynamic team of dedicated and experienced practitioners. We provide comprehensive, on-site medical care to a diverse range of patients in post-acute rehabilitation, long-term care, and assisted living centers.

Key Responsibilities:
  • Collaborate with a seasoned team of Advanced Practice Providers who provide ongoing and primary management of patients
  • Provide care for subacute and long-term care patients at the facility, seeing approximately 15-20 patients per week, per facility.
  • Perform regulatory visits and oversee new patient admissions in the subacute rehabilitation unit once per week per facility.
  • Participate in rehospitalization, acute care transfer and quality assurance meetings to maintain and improve patient care
  • Complete required documentation, including patient admission and progress notes, which may be done remotely from home
Requirements:
  • MD or DO with a valid Maine medical license
  • Board certified or board eligible in Internal Medicine, Family Medicine, Geriatrics, or a related field
  • Prior experience in subacute or long-term care settings is preferred
  • Strong communication and interpersonal skills
  • Commitment to providing high-quality patient care
  • Ability to work effectively in a collaborative, team-based environment
