Physical Therapist Assistant - Home Health Job Summary

We are seeking a highly skilled Physical Therapist Assistant to join our team in Ontario, CA. As a Physical Therapist Assistant, you will work closely with our physical therapists to provide high-quality care to patients in their homes.

Responsibilities include:

  • Providing physical therapy services to patients in their homes, under the supervision of a licensed physical therapist.
  • Conducting assessments and developing treatment plans to meet the unique needs of each patient.
  • Implementing treatment plans, including exercises, modalities, and other interventions, to promote patient recovery and independence.
  • Communicating effectively with patients, families, and healthcare team members to ensure seamless care coordination.
Requirements

To be successful in this role, you will need:

  • A current and valid California licensure as a Physical Therapist Assistant.
  • An Associate degree in physical therapy.
  • A minimum of one (1) year of experience as a Physical Therapist Assistant.
  • Basic Life Support (BLS) certification through the American Heart Association (AHA).
  • A valid driver's license and auto insurance.
