MDS Coordinator

We are seeking a highly skilled MDS Coordinator to join our team at Greencroft Communities. As a key member of our care planning team, you will play a vital role in ensuring that our residents receive the highest quality care.

• Care Planning: Develop and implement comprehensive care plans that meet the unique needs of our residents, in accordance with Federal guidelines.
• MDS Management: Complete and manage MDS assessments, ensuring timely and accurate completion.
• Collaboration: Work closely with our staff nurses to ensure seamless care planning and coordination.
• Resident Monitoring: Regularly monitor residents for significant changes and address clinical issues promptly.

• A licensed Registered Nurse or LPN in Indiana with previous nursing and Long-Term Care experience.
• Strong understanding of MDS and care planning principles.
• Excellent communication and interpersonal skills, with a focus on teamwork and collaboration.
