The Registered Nurse Outpatient Program Coordinator will be responsible for coordinating patient care, communicating effectively with interdisciplinary teams, and maintaining accurate records of patient progress and treatment plans. This role requires strong organizational and time management skills, as well as excellent communication and interpersonal abilities.
Key Responsibilities
* Coordinate patient care, including scheduling appointments and lab tests
* Communicate effectively with interdisciplinary teams to ensure seamless care coordination
* Maintain accurate and detailed records of patient progress and treatment plans
* Participate in staff training and education programs to enhance clinical knowledge and skills
* Assist with administrative tasks, such as data entry and filing
Requirements
* Graduate of an accredited School of Nursing
* Entry-level position for RNs with less than 1 year of Nephrology Nursing experience in the last two years as a RN
* Strong organizational and time management skills
* Excellent communication and interpersonal abilities
* Basic computer skills and familiarity with electronic medical records systems
