Job Description

As a Case Manager at Hope Network New Passages, you will be responsible for coordinating all services necessary to maximize consumer recovery and independence for individuals with mental illness. This includes implementing strategies for obtaining services and supports that are goal-oriented and individualized.

You will also be responsible for facilitating the implementation of Individual Plans of Service, performing comprehensive psychosocial assessments, and participating in legal proceedings including petitioning and testifying in clinical emergencies.

In addition, you will provide community-based services, facilitate the coordination of consumer intakes, discharges, and transfers, communicate with all service providers in Treatment Plans, and facilitate hospitalizations and attend hospital meetings.

Required Skills and Qualifications

To be successful in this role, you will need a Bachelor's degree in a human services field from an accredited institution, preferably with prior case management experience in a mental health or rehabilitation setting.

You will also need prior experience working with Developmentally Disabled populations and a valid Michigan Driver's License.
