Fairview Health Services Rehabilitation provides a wide array of services catering to patients across multiple healthcare settings. Our offerings include Physical, Occupational, and Speech Therapy, as well as Audiology and Cardiac & Pulmonary Rehabilitation. Our therapists work collaboratively with colleagues in various medical environments and participate in numerous specialized programs.

As an academic health organization with residency and fellowship opportunities, we foster a collaborative culture focused on continuous learning and evidence-based, patient-centered care. Our continuing education program offers over 40 courses annually at no cost to employees, along with support for external continuing education opportunities.

As a member of the Fairview rehabilitation team, you will have access to formal mentorship, clinical specialization, and recognition through our clinical specialist program. Our nationally recognized programs position our therapists as vital contributors to helping patients achieve their rehabilitation goals and enhance their quality of life.

Key Responsibilities

The Physical Therapist collaborates with patients and their families to conduct evaluations and re-evaluations. This includes gathering and applying relevant data, performing assessments, and determining treatment diagnoses.

Develops Customized Care Plans

Analyzes data to identify patient needs and potential risks, formulates recovery prognoses, and sets individualized objectives.

Implements and adjusts care plans based on ongoing assessments and patient responses.

Coordinates care and communicates effectively with the interdisciplinary team to ensure comprehensive care and appropriate discharge planning.

Provides Skilled Interventions

Modifies treatment strategies to reflect changes in patient status and responses to interventions.

Evaluates patient and caregiver educational needs and delivers suitable instruction.

Documentation and Billing

Utilizes appropriate medical terminology and adheres to professional practice standards.

Completes documentation promptly in accordance with departmental guidelines.

Ensures compliance with departmental billing protocols.

Professional Expectations

Effectively prioritizes tasks and patient care for optimal time management.

Understands and adheres to professional licensure and certification requirements.

Mentors team members and supervises students as assigned.

Engages in professional development to uphold clinical standards.

Quality Improvement Initiatives

Contributes to the development and implementation of quality enhancements and participates in organizational initiatives.

Qualifications

Required Licensure/Certification

Current Physical Therapy registration or licensure in Minnesota or a temporary license.

Valid Basic Life Support (BLS) certification.

Preferred Education & Experience

Doctorate in Physical Therapy.

Minimum of one year of experience in Physical Therapy practice.
