We are seeking a skilled and compassionate Hospice Nurse to join our team at Supplemental Health Care. As a Hospice Nurse, you will provide high-quality patient care and support to individuals in their homes, working closely with our partnering home healthcare providers in Wisconsin.

Key Responsibilities:

  • Provide comprehensive nursing care to patients in their homes, including assessment, planning, implementation, and evaluation of care.
  • Develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients and their families.
  • Collaborate with interdisciplinary teams, including physicians, social workers, and other healthcare professionals, to ensure seamless care coordination.
  • Conduct regular patient assessments and updates, documenting all interactions and care provided in the patient's medical record.
  • Participate in ongoing education and training to stay current on best practices and industry developments.

Requirements:

  • Current Wisconsin RN License
  • Minimum of 1 year recent relevant nursing experience
  • Electronic Charting experience
  • American Heart Association BLS
  • Valid U.S. Driver's License
