Mass General Brigham is seeking a skilled Nurse Case Manager to facilitate effective care coordination for patients admitted to Medical, Surgical, Pediatric, or Obstetrical Services. This role requires collaboration with healthcare teams and patients to develop discharge plans, ensuring seamless transitions of care.

About the Role

The ideal candidate will possess strong clinical knowledge, excellent communication skills, and the ability to work independently in a fast-paced environment. As a Nurse Case Manager, you will be responsible for:

  • Coordinating post-acute care services, including hospital outpatient areas and emergency department referrals.
  • Developing and implementing individualized discharge plans, considering patient needs, health status, and insurance coverage.
  • Collaborating with physicians, nurses, and other healthcare professionals to ensure appropriate level of care and compliance with regulations.

To succeed in this position, you must have a strong foundation in nursing practices, with a focus on case management principles. Your expertise will enable you to navigate complex healthcare systems, identify opportunities for improvement, and provide exceptional patient care.

Requirements and Qualifications
  • Bachelor's degree in Nursing (BSN) preferred; current licensure as a Registered Nurse in Massachusetts required.
  • Minimum three years' experience in general acute care hospitals, healthcare reimbursement, quality improvement, and enhanced models of healthcare delivery.
  • Prior experience in care coordination/discharge planning, utilization review, and hospital operations is essential.
  • CCM or ACMA certification preferred, but not mandatory.
