As a Physical Therapist with Geisinger Home Health, you will have the opportunity to work with patients in their homes, providing personalized care and support to help them achieve their goals.
Responsibilities- Evaluate patient functional status and physical therapy needs, developing a treatment plan to improve and enhance their well-being.
- Consult with physicians to develop therapy plans of care and provide services within the scope of practice as defined by state laws.
- Observe and report patient responses to treatment and any changes in their condition to the nurse supervisor and/or physician.
- Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice and evidence-based techniques.
- Instruct and inform participating members of the healthcare team, patients, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
- Current Physical Therapy licensure in the state of PA
- Current CPR certification
- Must have a current driver's license and vehicle insurance, and access to a dependable vehicle
