Job Summary

We are seeking a skilled Registered Nurse to join our team at Island Home Care in Key West. As a PRN RN, you will provide high-quality nursing care to patients in their homes, working collaboratively with our healthcare team to achieve exceptional patient outcomes.

Key Responsibilities
  • Initiate, develop, and implement comprehensive care plans in collaboration with physicians and other healthcare professionals.
  • Directly supervise and provide instruction to home health aides and licensed practical vocational nurses, ensuring compliance with state and federal regulations.
  • Accurately and timely complete comprehensive assessments, including medication reconciliation, and document patient visits according to policy and payer requirements.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Participate in quality improvement initiatives and adhere to Island Home Care's utilization management model.
Requirements
  • Current RN licensure in the state of practice
  • Current CPR certification
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
