Key Responsibilities:
  • Provide one-to-one patient care in a safe and comfortable home environment.
  • Assess patients for stability and follow through with all doctor's orders.
  • Verify medication milligrams, last day dosed, take outs received, phase level, and verify guest doses.
  • Document patient information accurately and thoroughly.
  • Collaborate with patients and their families to provide education and support.
Requirements:
  • High school diploma or equivalent and graduate of an accredited LPN/RN program.
  • Current licensure as an LPN/RN in the state of practice.
  • At least 12 months of hands-on nursing experience.
  • Current CPR certification.
  • Ability to work independently and as part of a team.
