We are seeking a highly skilled Palliative Medicine LCSW to join our team at Carle Health. As a Palliative Medicine LCSW, you will play a critical role in providing interventions to patients with complex psychosocial needs.

Key Responsibilities:
  • Assess and address patient/family psychosocial needs through evaluation of prior functioning levels, support systems, reaction to illness, and ability to cope.
  • Counsel patients/families regarding emotional, social, and financial consequences of illness or disability.
  • Access and mobilize family/community resources to meet identified needs.
  • Collaborate with healthcare providers and community agencies to promote and encourage each person's contributions toward achieving optimal and realistic patient goals.
Requirements:
  • Bachelor's Degree in Social Work or Master's Degree in Social Work.
  • Licensed Clinical Social Worker (LCSW) in Illinois.
  • Ability to coordinate multiple projects concurrently and establish priorities.
  • Strong planning and organizational skills.
  • Self-motivated and flexible in hours and job duties.
