As a RN Emergency Care Nurse Coordinator at Yale New Haven Health, you will play a critical role in delivering high-quality patient care and navigation through the healthcare system. Key responsibilities include:
- Collaborating with patients, families, and interdisciplinary teams to assess needs, develop care plans, and coordinate services.
- Ensuring seamless transitions between care settings, facilitating timely access to necessary treatments and resources.
- Communicating effectively with physicians, mid-level providers, and other stakeholders to ensure accurate information exchange and informed decision-making.
- Bachelor's Degree in Nursing (BSN) or Master's Degree in Health-related field preferred.
- A minimum of three years' experience in adult inpatient services, ambulatory oncology, or operating room setting.
- Connecticut RN license; Oncology Nursing Society certification (OCN, AOCN) desirable.
