Job Title: Home Health Occupational Therapist

LHC Group is seeking a skilled Home Health Occupational Therapist to join our team in Roseland, New Jersey.

Job Summary:

We are looking for a dedicated and compassionate Occupational Therapist to provide high-quality care to our patients in the comfort of their own homes. As a Home Health Occupational Therapist, you will be responsible for assessing and evaluating patient care needs, developing treatment plans, and implementing interventions to improve functional status and overall well-being.

Responsibilities:
  • Evaluate patient's functional status, occupational therapy needs, and status of all body systems as required for CMS documentation.
  • Develop and implement treatment plans, including interventions aimed at improving and enhancing patient's well-being.
  • Consult with physicians in the development of therapy plans of care.
  • Provide education and training to patients, families, and healthcare teams on occupational therapy techniques and strategies.
  • Document patient progress and outcomes in accordance with LHC policy and regulatory requirements.
  • Participate in staff conferences and committees as requested by the agency.
Requirements:
  • Current Occupational Therapy licensure in the state of New Jersey.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to adapt to changing situations and priorities.
