CVS Health is seeking a highly skilled and compassionate Nurse Practitioner to join our Primary Care team. As a key member of our clinical care team, you will provide high-quality, patient-centered care to a panel of patients, working collaboratively with a dedicated team to drive better outcomes.
Responsibilities:- Provide accurate and complete assessments, diagnoses, treatments, and management of health problems, health counseling, and disposition planning.
- Collaborate with the clinical care team to ensure the delivery of high-quality, patient-centered, longitudinal care.
- Communicate effectively with members of the clinical care team, other health professionals, and health-related agencies.
- Participate in and lead clinical care team huddles, promoting mutual respect and trust within the team.
- Work effectively with the clinical care team to drive better outcomes for patients.
- Master's degree in Family Nurse Practitioner with current National Board Certification and State of Employment license to practice in the Advanced Practice role.
- A minimum of 1 year experience as a Nurse Practitioner, with experience in Family medicine or primary care practice preferred.
- Active Advance Practice Registered Nurse, Nurse Practitioner license in good standing.
- Current National Board Certification in Family Practice.
- Competitive pay range: $99.14 per hour.
