Job Summary

We are seeking a highly skilled Clinical Pharmacist to join our team at Trillium Health. As a Clinical Pharmacist, you will play a critical role in providing direct patient care and collaborating with healthcare teams to optimize patient outcomes.

Key Responsibilities
  • Provide direct patient care through collaborative drug therapy management (CDTM) for clinic patients, including chronic disease state management and medication reconciliation.
  • Collaborate with pharmacy team and provider team to ensure access to medications and improve the stewardship of limited healthcare resources.
  • Participate in day-to-day clinical pharmacy operations through collaborative practice agreements and participate in the day-to-day operation of pharmacy activities.
  • Perform non-dispensing functions, including prescription processing, accurate patient billing, and inventory management.
  • Provide education to patients, providers, and medical staff on chronic disease states and advocate for the use of clinic and pharmacy services.
  • Precept other pharmacy residents and students.
Requirements
  • Doctor of Pharmacy or BS Pharmacy degree.
  • Completion of PGY 1 residency or 3 years clinical experience.
  • Current NYS pharmacist license with immunizer credential.
  • Current NYS Collaborative Drug Therapy Management credential, or ability to obtain within 3 months of hire.
  • Minimum of 1 year experience in primary care pharmacy involving direct, collaborative patient care.
  • Experience precepting students and/or residents.
Preferred Qualifications
  • Current Board of Pharmacy Specialties credential.
  • Current American Academy of HIV Medicine Pharmacist (AAHIVP) credential.
  • Certified Diabetes Educator (CDE) or Board Certified-Advanced Diabetes Management (BC-ADM).
  • Completion of PGY 2 residency in primary care, ambulatory care or similar experience.
  • Experience working in community-based healthcare setting.
