Nurse Case Manager

About the Role:We are seeking a skilled and compassionate Registered Nurse to join our team as a Case Manager. In this role, you will be responsible for conducting in-home nursing visits for our clients, who are former Nuclear Weapon's Workers with chronic/terminal illness due to exposure to toxic substances while working for the Government.
Key Responsibilities:- Conduct in-home nursing visits for clients, including physical assessments, ensuring home safety, and overseeing skilled and non-skilled caregivers.
- Provide case management to assigned clients, including planning, implementing, and evaluating care provided.
- Collaborate with staffing coordinators to ensure the appropriateness of staffing and scheduling of personnel within scope of practice, competencies, client needs, and complexity of home care.
- Adhere to nursing delegation guidelines and ensure adherence to Agency policies.
- Participate in all Agency performance improvement initiatives, including quarterly medical record review.
- Graduate of a state-approved school of professional registered nursing.
- Minimum of two (2) years nursing experience, including one (1) year in home care or a closely related field.
- Competitive hourly rate of $00 plus shift differentials, where applicable.
