About the Role

We are seeking an experienced Occupational Therapist to join our team. As a key member of our interdisciplinary team, you will work closely with patients, families, and healthcare professionals to develop and implement customized rehabilitation plans. Your expertise will help patients overcome physical, emotional, and cognitive challenges, enabling them to live healthier and happier lives.

Key Responsibilities:

  • Confer with patients' physicians and other healthcare team members to develop and revise treatment plans.
  • Review and expand retraining programs to meet patients' behavioral goals.
  • Report patient status and progress to physicians, rehabilitation staff, Clinical Managers, patients, and family members.
  • Participate in care coordination and discharge planning activities, acting as a resource to other healthcare personnel.
  • Design community reintegration activities to assist clients in physical reconditioning, psychological adjustment, and social reintegration.
  • Recommend and design adaptive equipment to improve residents' working environments or participation in the rehabilitation program and community.
  • Train staff and family members to carry out retraining programs, including dressing, feeding, grooming, and hygiene skills.
  • Ccoordinate/oversee/supervise/instruct and evaluate Occupational Therapy Assistant and Home Health Aide performance in implementing occupational therapy services.
  • Accurately document patients' care observations, interventions, and evaluation on the day services are rendered.

Requirements:

  • Degree from an accredited Occupational Therapy Program.
  • A minimum of six months of occupational therapy experience preferred.
  • Home Health experience a plus.
