Estimated Salary: $65,000 - $90,000 per year, depending on experience.

Job Description:

The Physical / Occupational Therapist will provide in-home services to clients, conducting assessments and developing personalized treatment plans to enhance their functional abilities and quality of life.

Responsibilities:

  • Conduct comprehensive assessments of patients' physical and occupational needs.
  • Develop and implement individualized treatment plans, incorporating evidence-based practices and interventions.
  • Treat patients to relieve pain, develop and restore function, and maintain maximum performance using appropriate physical therapy treatments, modalities, and procedures as stated in the physical/occupational therapy plan of care.
  • Monitor and record patients' progress, adjusting treatment plans as necessary.
  • Collaborate with multidisciplinary teams, including physicians, nurses, and other healthcare professionals, to ensure coordinated care.
  • Instruct and advise patients, families, and other health team personnel on phases of physical/occupational therapy where they may assist patients.
  • Ensure compliance with established standards of practice, therapy department procedures, and productivity standards.

Required Qualifications, Skills, and Knowledge:

  • Degree from an accredited Physical/Occupational Therapy Program.
  • Current professional licensure as a Physical Therapist or Occupational Therapist license eligible based on the rules and regulations of the state in which you are applying for role.
  • Current CPR Certification.
  • Strong written and oral communication skills, as well as good organizational ability.
  • Computer literacy, with proficiency in standard business software applications.

Working Conditions:

Centria's office hours are Monday through Friday from 8:30am-6:00pm. Additional time may be required to complete work or meet company objectives.
