Responsibilities
- Clinical Competence
- Develop and implement comprehensive care plans in collaboration with physicians and other healthcare professionals.
- Supervise and direct the care provided by home health aides and licensed practical vocational nurses, ensuring adherence to state and federal regulations.
- Documentation and Care Delivery
- Provide high-quality clinical services within the scope of practice, adhering to infection control standards and payer requirements.
- Accurately and timely complete comprehensive assessments, including medication reconciliation.
- Document patient visits and care delivery in accordance with agency policies and payer requirements.
- Quality
- Ensure patients meet home health eligibility and medical necessity guidelines, accurately determining primary focus of care and developing plans in collaboration with physicians.
- Communicate effectively with agency staff, physicians, patients, and family members to ensure quality care and service excellence.
- Professionalism and Teamwork
- Exhibit a positive attitude, willingness to learn, and adaptability in a fast-paced environment.
- Participate in on-call rotation, weekend rotation, and other duties as assigned to meet patient needs.
Qualifications
License Requirements
- Current RN licensure in the state of practice.
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
Additional State Requirements
- CA: One year prior professional nursing experience.
- LA: At least one year of clinical experience as a Registered Nurse, or one year of clinical experience as an LPN with recent experience.
- AL, AR, AZ, CO, CT, FL, GA, ID, IL, IN, KY, MA, MD, MS, MI, MO, NC, NH, NJ, NY, NV, OH, OK, OR, PA, RI, SC, TN, TX, VA, WA, WI, WV: No additional state-specific requirements.
