Hospice Nurse Practitioner Job Summary

We are seeking a compassionate and skilled Hospice Nurse Practitioner to join our team at Self-Help for the Elderly. As a key member of our hospice care team, you will provide high-quality patient care and support to individuals and their families in their homes.

Key Responsibilities:
  • Conduct face-to-face visits with hospice patients to assess their physical condition and determine eligibility for continuation of hospice services.
  • Communicate effectively with attending physicians, hospice medical directors, and hospice team members to coordinate optimal care and resource utilization.
  • Obtain and document patient data on physical, psychological, social, and spiritual factors that may influence their health status.
  • Provide detailed written narratives when clinical findings support a life expectancy of six months or less.
  • Maintain accurate and timely documentation of all care delivered, ensuring compliance with local, state, and federal statutes and regulations.
Requirements:
  • Current California Licensed Nurse Practitioner.
  • One year of prior professional nursing experience, with home health, oncology, or hospice experience preferred.
  • Understanding of hospice principles of care and philosophy, with familiarity with hospice requirements.
  • Current CPR certification.
  • Ability to travel via public transportation or personal vehicle for performance of job duties.
