Registered Nurse

We are seeking a compassionate and skilled Registered Nurse to join our Home Health team in Chapel Hill, NC. As a Home Health RN, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to ensure optimal outcomes.
Responsibilities- Provide safe and timely patient care using evidence-based nursing practices and best practices as defined by our quality assurance team.
- Participate in weekly team meetings to coordinate patient care and ensure compliance with COVID-19 safety measures.
- Develop and implement individualized nursing care plans to facilitate patient rehabilitation and achieve quality outcomes.
- Provide medication instruction, monitoring, and administration as ordered by the patient's Primary Care Physician.
- Conduct patient assessments and collaborate with the interdisciplinary team to develop and update patient care plans.
- Supervise Licensed Practical Nurses and Home Health Aide staff as needed.
- Document patient care and progress in a timely and accurate manner, adhering to state and federal regulations.
- Registered Nursing Degree (Associate or Bachelor) from an accredited college of nursing with current unrestricted registration and license in the applicable state.
- One year of nursing practice in a patient care setting, with experience in home health, geriatrics, or related settings preferred.
- Valid driver's license, acceptable driving record, and proof of car insurance in accordance with our company policy.
- Current CPR certification.
