About the Role

As a Case Manager at Mercy Rehabilitation Hospital – Springfield, you will play a vital role in the interdisciplinary team providing care to patients recovering from strokes, brain or spinal cord injuries, amputations, complex orthopedic injuries, and other conditions. Our hospital is accredited by The Joint Commission (TJC) and the Commission on Accreditation of Rehabilitation Facilities (CARF) for inpatient rehabilitation.

Key Responsibilities

• Collaborate with the patient care team to develop, monitor, and refine treatment plans to achieve identified patient goals and outcomes.
• Complete documentation per workflow timeline and content requirements, including the Individual Plan of Care (IPoC) per CMS guidelines.
• Schedule family conferences and communicate with caregivers to keep patients and designated caregivers informed of progress and provide appropriate information related to goal achievement, course of rehabilitation stay, and plans for discharge.
• Coordinate weekly patient care team conferences to facilitate development, monitoring, and refinement of treatment plans.
• Review the patient's assigned CMG and identify any potential missed comorbid conditions that are actively being treated during the patient's stay. Communicate any findings to the HIM team.

Requirements

• Current Registered Nurse or Social Work licensure or Healthcare professional licensure as Respiratory Therapist, Physical Therapist, Speech Language Pathologist, or Occupational Therapist.
• Certification in Case Management or Rehabilitation Nursing preferred.
• Minimum of 2 years social work or case management experience in an inpatient setting highly preferred; acute/rehabilitation hospital experience preferred.
• Effective oral and written communication skills in English, additional languages preferred.
• Basic computer skills in Excel, Word, Outlook, PowerPoint, etc. required.
