Our hospice program is seeking a dedicated RN Field Nurse/Case Manager to become part of our compassionate team. Being a smaller program, you will have the unique opportunity to offer personalized care to your patients.

Role Overview: The Hospice RN Case Manager is an integral member of the interdisciplinary team, operating under the general guidance of the Medical Director and/or the attending physician. This role involves skilled nursing assessments, planning, and coordination of services aimed at enhancing the comfort and well-being of patients and their families.

Essential Qualifications:

  • Graduation from a nursing program accredited by the National League of Nursing; a Bachelor’s degree in nursing is preferred.
  • Possession of a valid driver's license, current auto insurance, and access to a vehicle during work hours.
  • Experience in hospice or home health care with terminally ill patients is highly desirable.

Key Skills and Competencies:

  • Demonstrated commitment to providing quality services.
  • Proficient nursing skills, ability to manage emotional stress of patients and families, and respect for diverse lifestyles.
  • Basic computer proficiency.

Job Types: Full-time, Part-time, Per diem
