About the Position

As a PRN Home Risk Assessment provider, you will work collaboratively with physicians, case managers, and other healthcare professionals to identify and address patients' health risks. Your expertise will help improve quality of care, close care gaps, and enhance patient outcomes.

Responsibilities
  • Conduct comprehensive health assessments in patients' homes, reviewing medical history, medication adherence, social environment, and other risks.
  • Collaborate with healthcare professionals to develop personalized care plans and provide education to patients and their families.
  • Work independently and as part of a team to ensure seamless care coordination and communication.
  • Document patient interactions and assessments accurately and thoroughly, adhering to Matrix Medical Network's guidelines and protocols.
Requirements
  • Masters Degree in Nursing or Physician Assistant Studies, or equivalent experience and licensure.
  • Current RN and NP/PA licensure in the state of practice, with prescription authority or ability to obtain.
  • Board certification in a Matrix-approved specialty.
  • Minimum 1 year experience as a Nurse Practitioner or Physician Assistant.
  • Strong computer skills and familiarity with EMR software.
  • Excellent verbal and written communication skills, with a positive and outgoing personality.
