At Abode Care Partners, we are seeking a highly skilled Nurse Practitioner to join our medical team. This rewarding role involves providing personalized care to seniors and patients with complex needs in various settings.
The ideal candidate will have a strong background in evidence-based practices and excellent interpersonal communication skills. As a Nurse Practitioner at Abode Care Partners, you will play a vital role in ensuring high standards of healthcare through collaborative work with other healthcare professionals.
Responsibilities:- Provide comprehensive medical care to patients, including chronic and urgent episodic illnesses, minor injuries, and accidents.
- Collaborate with caregivers and other healthcare professionals to develop and maintain comprehensive patient plans.
- Manage patient medical records, ensuring accurate and up-to-date information.
- Develop and implement treatment plans based on patient needs and medical histories.
- Active state APRN licensure
- Current National Board Certification as an APRN
- Active DEA licensure
- Master's degree in Nursing (MSN)
- Five or more years of nursing experience; at least two years of APRN experience preferred
- Excellent time management skills and ability to prioritize tasks effectively
- Demonstrated proficiency in medical instruments and equipment
