Supplemental Health Care is seeking a skilled Registered Nurse to join our team as a Home Health RN. As a Home Health RN, you will provide high-quality patient care in the comfort of their own homes.

We are looking for a dedicated and compassionate RN to work with our partnering home health care provider in Hicksville, New York. As a Home Health RN, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities
  • Provide high-quality patient care in the comfort of their own homes
  • Conduct thorough assessments and develop individualized care plans
  • Collaborate with the interdisciplinary team to ensure seamless care coordination
  • Document patient progress and outcomes

We offer a competitive compensation package, including a weekly rate of $2,080 - $2,315, and a comprehensive benefits package, including medical, dental, vision, life, and pet insurance. Requirements
  • OASIS/Medicare Certified Visits Experience
  • Electronic Charting experience
  • Minimum of 1 year of recent home health nursing experience (private duty does not qualify)
  • Most contracts require some on-call
  • Current New York RN License
  • American Heart Association BLS
  • Valid U.S. Driver's License
