LPN/LVN (Part Time) - Cully Assisted Living Position

Job Summary:
This position will be working in Cully Assisted Living. As a Licensed Practical Nurse (LPN) with Providence Health & Services, you will provide quality nursing care in a clinic or community setting. You will work under the direction of a Registered Nurse and function within the Interdisciplinary Team (IDT) to formulate and evaluate the Participant's Plan of Care (POC).
Responsibilities:
- Completes focused assessments, by collecting both subjective and objective data.
- Assesses educational needs and develops trainings for Participants, families and caregivers.
- Identifies areas of need or potential change in a Participant's status.
- Performs clinical tests, treatments, therapies, medication administration, health teaching and counseling, as directed by the PCP and RN.
Requirements:
- Education to meet certification, license or registration requirement.
- Oregon Licensed Practical Nurse.
- National Provider BLS - American Heart Association within 30 days of hire.
