Home Health Physical Therapist Role

We are seeking a skilled and compassionate Home Health Physical Therapist to join our team at Methodist Healthcare at Home.
As a member of our healthcare team, you will provide high-quality patient-centered care by managing the complete process of therapeutic care for an assigned group of patients in coordination with the physician and the patient care team.
The ideal candidate will be a licensed Physical Therapist with a Bachelor's degree and a valid driver's license. No experience is required, but a strong passion for creating positive patient interactions and a commitment to caring for the well-being of others is essential.
In this role, you will assess and evaluate therapeutic/rehabilitative/functional status and participate in the development of the total plan of care. You will also provide purposeful and skilled therapeutic care at patients' homes, using procedures and techniques to produce a change in condition/function consistent with the physician's plan of care.
You will instruct patients/families/caregivers in the use of therapies, exercises, therapeutic appliances, and/or prosthetic devices to be used outside of therapy sessions. Additionally, you will prioritize your caseload, determine daily staffing needs, and coordinate assignments/handoffs.
The salary for this position is estimated to be around $93,000 per year, depending on location and qualifications.
