Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team. As an LPN at Dominion Senior Living, you will provide compassionate care to residents in a comfortable and collaborative work environment. You will be responsible for conducting initial and ongoing assessments of residents' physical and emotional health, documenting changes in residents' conditions, and communicating with the healthcare team and dietary team.

Responsibilities
  • Conduct initial and ongoing assessments of residents' physical and emotional health
  • Document changes in residents' conditions and communicate with the healthcare team and dietary team
  • Collaborate with the healthcare team to develop and update individualized care plans based on residents' needs and preferences
  • Participate in care plan meetings to discuss progress and adjustments
  • Supervise and delegate tasks to CNAs and Resident Assistants
  • Provide guidance and direction to team members in performing their duties effectively and safely
  • Assist with activities of daily living, such as bathing, dressing, and grooming
  • Coordinate and perform medical procedures and treatments, such as wound care, injections, and monitoring of chronic conditions, under the guidance of the Wellness Director
  • Maintain detailed and up-to-date medical records, including residents' vital signs, medication administration, and changes in health status
Requirements
  • Completion of a state-approved practical nursing program
  • Graduation from a practical nursing program accredited by the state nursing board or a national accrediting agency
  • Professionalism in interactions with residents, families, and teammates
  • Clinical expertise in tasks such as medication administration, wound care, and monitoring vital signs
  • Teamwork and collaboration with registered nurses, CNAs, and other staff
  • Lifelong learning and commitment to ongoing education and training
