Position Overview

The role involves direct engagement with clients of the Assertive Community Treatment (ACT) Team, utilizing the evidence-based ACT model established by NAMI. The focus is on delivering comprehensive and holistic services that address various client objectives, including mental health, physical health, housing, employment, and social aspirations. Responsibilities include conducting assessments and formulating treatment plan goals in collaboration with clients and team members to achieve the outcome objectives set by Polk County Health Services (PCHS). Additionally, the position entails providing medication education and support for ACT clients and case managers, processing orders, completing prior authorizations, and coordinating with medication prescribers to ensure clients receive necessary psychiatric medications.

Key Responsibilities

Client Engagement: Assess client needs and establish goals to promote independence within the community. Provide skills training and collaborate with clients and team members to explore alternatives to hospitalization, higher levels of care, homelessness, and incarceration. Advocate for clients to meet their needs and interests while exercising critical thinking in independent decision-making. Maintain accountability for the services provided and demonstrate respect for clients with diverse abilities and diagnoses. Conduct home visits or meet clients in public spaces, with a valid driver's license and willingness to transport clients as needed. Exhibit teamwork skills such as problem-solving and cooperation, and be available for on-call shifts, including evenings and weekends, to manage crisis situations and deliver medications.

Medical Responsibilities: Under the guidance of the team medication prescriber, collaborate with registered nurses to develop, maintain, and oversee medication treatment policies. Evaluate and document the effectiveness of psychotropic medications, manage lab work, and ensure medication is integrated into individual treatment plans. Administer IM injections and follow protocols for medication management and disposal. Ensure client attendance and follow-up with psychiatric prescribers.

Documentation: Complete all required documentation in accordance with established guidelines, prioritizing client needs and adhering to funder and accreditation standards. Track and document monthly outcome objectives.

Qualifications

Education: Must be a registered nurse or licensed practical nurse, with a preference for candidates with mental health experience.

Experience: Minimum of one year working with the relevant population.

Licenses/Certifications: Valid driver's license for work-related driving; must be licensed as an RN or LPN in good standing in the state of Iowa.

Skills and Abilities: Proficient in speaking, reading, and writing fluent English. Ability to comprehend common mental health terminology and effectively utilize electronic health records and Microsoft Office. Capable of presenting information clearly and responding to inquiries from clients and professionals.

Other: Familiarity with standard equipment necessary for performing essential job functions.
