Job Overview

We are seeking a skilled Clinical Risk Assessment Specialist to join our team at Matrix Medical Network. This is a PRN/Part Time opportunity for a Nurse Practitioner or Physician Assistant to conduct comprehensive health assessments in the comfort of patients' homes.

Key Responsibilities:

  • Conduct thorough adult and geriatric assessments, including medical history, diagnosis, treatment, health education, and referrals as needed
  • Collaborate with physicians, case managers, social workers, and other healthcare professionals to ensure seamless care coordination
  • Provide expert guidance on patient education, follow-up care, and medication management
  • Travel to patients' homes, skilled facilities, and other community settings to deliver high-quality care

Requirements:

  • Master's degree in Nursing or Physician Assistant studies, OR commensurate experience and licensure
  • Current RN and NP or PA licensure, with prescription authority and BLS, ACLS, or CPR certification
  • Board certification in a relevant specialty, with strong computer skills and EMR software familiarity
  • Minimum 1 year experience as a Nurse Practitioner or Physician Assistant, with excellent verbal and written communication skills
  • Able to travel within state or out of state, with a valid driver's license and proof of adequate automobile insurance
