Job Description

As a Hospice Nurse Specialist, you will be responsible for delivering high-quality patient care and coordination services to meet the unique needs of each individual. Your role will involve:

  • Conducting comprehensive assessments of patients and their families to identify care needs
  • Developing and implementing personalized care plans that address physical, emotional, and spiritual needs
  • Collaborating with healthcare professionals to ensure seamless care transitions and effective communication
  • Providing education and support to patients and their families on disease management, symptom control, and end-of-life care

You will work closely with our interdisciplinary team to provide holistic care and support to our patients and their families. Requirements

To be successful in this role, you must have:

  • A current Registered Nurse (RN) license in the state of California
  • 1-2 years of experience in hospice or palliative care, with a focus on home care or acute settings
  • Excellent observation, verbal, and written communication skills
  • Basic math skills and ability to manage medications and treatments
