We are seeking a highly skilled Registered Nurse to join our team. As a Registered Nurse, you will be responsible for providing high-quality patient care and services to individuals with diverse needs.
Key Responsibilities- Monitor and assess patients' medical and health needs, providing individualized care and services.
- Develop and implement comprehensive care plans, ensuring timely and effective delivery of services.
- Collaborate with healthcare professionals, families, and caregivers to ensure seamless care coordination.
- Provide education and training to patients, families, and caregivers on health-related topics and disease management.
- Conduct regular home visits to provide nursing oversight and support, ensuring patients receive the necessary care and services.
- Document patient progress, outcomes, and any concerns or issues, maintaining accurate and up-to-date records.
- Participate in quality improvement initiatives, contributing to the development and implementation of best practices and policies.
- Current licensure as a Registered Nurse in New York State.
- Minimum of 2 years of experience in a healthcare setting, preferably in a similar role.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Strong problem-solving and critical thinking skills, with the ability to analyze complex situations and develop effective solutions.
- Ability to work independently and as part of a team, with a strong commitment to patient-centered care.
