">We are seeking a skilled Nurse Practitioner to join our team in Redwood City, California. As a key member of our healthcare team, you will conduct home risk assessments, review medical histories, and provide personalized care to our patients. ">Responsibilities
">- Conduct adult and geriatric assessments, reviewing medical history, diagnosing, treating, educating, referring physicians, and managing cases according to Matrix guidelines.">
- Collaborate with physicians, case managers, social workers, family members, caregivers, and other medical personnel as needed.">
- Work closely with Primary Care Physicians (PCPs) to educate patients, provide follow-up support, and enhance patient outcomes.">
- Provide services in various settings, including home visits, skilled facility visits, and telehealth visits.">
Requirements
">- A Master's degree in Nursing or equivalent experience and satisfactory completion of NP licensure.">
- Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain it.">
- Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty.">
- A valid BLS, ACLS, or CPR certification.">
- A minimum of 1 year of experience as a Nurse Practitioner.">
- Strong computer skills and familiarity with employee health/medical record software.">
- Excellent verbal and written communication skills with patients, clients, and colleagues.">
- Comfortable and flexible with frequent changes.">
Travel Requirements
">- Valid state driver's license, proof of adequate automobile insurance coverage, and ability to drive a car.">
- Ability to travel to meet patients where they live.">
Compensation and Benefits
">The salary for this position is $115 per completed assessment. We also offer a mileage reimbursement plan and limited voluntary benefits, including a 401(k) plan. As a PRN/Part Time provider, you will enjoy flexible scheduling and competitive compensation.
">Location and Schedule
">This role is based in Redwood City, California, and offers flexible scheduling with no on-call, evening, or weekend hours unless desired. The primary location is Redwood City, CA 94063, with opportunities to work from home and in other community environments.
