Job Summary:

As a skilled nursing professional with Community Hospice of Northeast Florida, you will provide high-quality patient care in a variety of settings, including home care. Under the supervision of the Triage manager and the direction of the Triage supervisors or Team managers, you will assess patient needs, manage symptoms, and provide emotional support to patients and caregivers. Your work hours will include every Saturday and Sunday from 8am-8pm and one weekday (either Monday or Friday) from 8:30am-5pm, based on organizational needs.

Key Responsibilities:

Perform skilled nursing activities such as assessment, symptom management, and treatment in accordance with the plan of care and physician's orders.
Provide information and education regarding disease progression and anticipated needs at end of life.
Collaborate with patient, family/caregiver, and practitioner regarding goals of care and changes in treatment plan.
Follows precautions based on plan of care.
Provides appropriate emotional support during all conversations with patients/caregivers.

Requirements:

Possession of a degree from a diploma nursing program or a Bachelor's degree in Nursing.
Minimum of two (2) years experience in acute care setting, oncology, or home health care setting.
Skilled at teaching, communication and listening skills, assessment skills, central/peripheral IV skills, working within the framework of a multidisciplinary team.

Physical Requirements/Work Environment:

Home care position for 8 to 12 hour shifts. Use of own vehicle required.
