Key Responsibilities
  • Provide leadership and support to staff on shift, ensuring a safe and effective work environment.
  • Develop and implement individualized care plans, working closely with consumers, families, and healthcare professionals.
  • Collaborate with the supervisor to ensure seamless operations, including scheduling, training, and compliance.
  • Conduct daily observational data and document using electronic records, adhering to Agency standards.
  • Transport consumers safely, report vehicle maintenance needs, and maintain an accident-free driving record.
  • Administer and document prescribed medications, treatments, and health concerns, following Agency protocols.
  • Intervene in crisis situations, promoting safety and well-being for all individuals involved.
  • Provide direct care for individuals with varying needs, including bathing, hygiene, skin care, toileting, bowel and bladder programs, feeding, dressing, and transfers.
  • Maintain a safe working environment, using proper lifting techniques, positioning individuals safely, and addressing safety issues proactively.
Requirements
  • Bachelor's degree with 2 years of experience in a relevant setting, or Associate's degree with 3 years of experience, or HS Diploma/GED with 4 years of experience.
  • Demonstrated critical thinking and problem-solving skills to respond to medical and behavioral issues.
  • Leadership skills, ability to train and mentor staff, and a commitment to delivering high-quality services.
  • Ability to work with diverse populations, including individuals from various religious, ethnic, and social-economic backgrounds.
  • Valid driver's license and acceptable driving record, preferred.
  • Ability to complete all required training, including annual trainings and professional development.
  • Basic computer skills and ability to communicate effectively in written, verbal, and electronic formats.
